Athletic Physical Form and Parentvue

In order to be eligible for the Spring 2026 Track and Field Season, all athletes must have a current physical exam and complete the athletic registration process in Parentvue. The physician who performs the physical exam must fill out the Cobb County Physical exam forms. I've attached them to this post. Please print them out and take them to the doctor with you. Also, the forms and other required documents must be uploaded to Parentvue or they do not count. I've attached the instructions for how to do that here as well.
